Job Summary: Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient’s healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Job Requirements: Education and Work Experience: Bachelor’s Degree in Nursing (BSN): Preferred One year of professional nursing experience: Required Experience in a care management role: Preferred Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required Case management certification: Preferred Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required Essential Functions: Leads the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care. Ensures the interdisciplinary care plan is consistent with the patient?s clinical course, continuing care needs and covered services by monitoring diagnostic testing, treatments and procedures, and other aspects of patient care as appropriate for acute care. Discusses with physicians, the appropriateness of resource utilization, consultations, treatment plan, estimated length of stay and discharge plan. Focuses on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients. Collaborates with acute care case managers to ensure appropriateness on ongoing care. Coordinates the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services. Provides oversight and collects data required for regulatory/accreditation compliance. Manages frequent emergency department visitors by conducting a focus study review of the previous and current admissions. Performs other job-related duties as assigned.

Essential Responsibilities: ” Utilization Management: Performs daily pre-admission, admission, and concurrent utilization reviews using guidelines, institutional policies/procedures, and other information to determine appropriate levels of care and readiness for discharge. ” Monitors the progression of the discharge plan and facilitates discussions with the multidisciplinary teams. ” Educates other healthcare team members on utilization and care coordination. ” Collaborates with and provides information to patients, families, physicians, and staff regarding the discharge plans and transitions. ” Understands and consistently applies the interpretation, and utilization of member health care benefits. ” Conducts UM, care coordination and discharge planning activities according to all applicable regulatory requirements. ” Discharge Planning: Ensures continuity of care through communication in rounds and written documentation, level of care recommendations, transfer coordination, discharge planning and obtaining authorizations/approvals as needed for outside services for the patient. ” Develops, evaluates, and coordinates a comprehensive discharge plan in conjunction with the patient/family, physician, nursing, social services, and other health care providers and agencies. ” Regulatory: Conducts UM, care coordination, and discharge planning activities according to all applicable regulatory requirements. ” Kaiser Permanente conducts compensation reviews of positions on a routine basis. A Brief Overview: Case Managers are licensed nursing professionals responsible for coordinating continuum of care and discharge planning activities for a caseload of assigned patients. Major responsibilities include coordinating all facets of a patient’s admission/discharge; performing utilization review activities, including review of patient charts for timeliness of services as well as appropriate utilization of services; and ensuring optimum use of resources, service delivery, and compliance with external review agencies’ requirements. Case Managers act as consultants to the clinical team, service lines, and other departments regarding patient assessment and patient care and participate in program development and quality improvement initiatives. In their role, Case Managers, by applying guidelines and collaborating with multidisciplinary teams, influence and direct the delivery and quality of patient care. A hospital-based case management system has as its primary goal to ensure the most appropriate use of services by patients and, toward that end, to avoid duplication and misuse of medical services, control costs by reducing inefficient services, and improve the effectiveness of care delivery. Objectives are to facilitate timely discharge; prompt, efficient use of resources; achievement of expected outcomes; collaborative practice; coordination of care across the continuum; and performance/quality improvement activities that lead to optimal patient outcomes. A Case Manager differs from other roles in professional nursing/health care practice in that it is not intended to provide direct patient care; rather, a Case Manager will be assigned to specific patients to ensure that the medical services and treatments required are accomplished in the most financially and clinically efficient manner.

Job Summary: Establishes, coordinates, and maintains the process to increase patient through-put to the most appropriate level of care while facilitating interdisciplinary care across the continuum for the Emergency Department (ED). Collaborates with the patient/family, multidisciplinary team, physicians, community resources and payers to ensure the patient?s progress and level of care is appropriately determined for the emergency episode of care and for the inpatient encounter to ensure that care is performed and facilitated in an efficient and cost-effective manner. Uses well developed knowledge and skills in patient status determination (InterQual Criteria), in the assessment and care management of patients and families within the ED, inpatient and outpatient settings. Practices patient/family assessment and management, resource management, identifying patients appropriate for inpatient admission, observation, or outpatient status, care facilitation, discharge planning with referral to all levels of care, and other related duties specific to the defined patient population. Collaborates with other case managers, social work staff, the access center, admissions, and other physician and administrative leadership staff within the ED, the care management department and both the inpatient and outpatient settings. Uses knowledge of pathophysiology, pharmacology, and clinical care processes to participate with other clinical staff and physicians in the development of clinical practice guidelines and physician order sets for the purpose of improving quality of care, changing practice, and reducing costs. Job Requirements: Education and Work Experience: Bachelor’s Degree in Nursing (BSN): Preferred Master’s Degree in nursing or health care related field: Preferred Experience in case management, utilization management, and/or transitional care: Preferred Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required Accredited Case Manager, Certified Case Manager, or Board Certification in Case Management: Preferred Essential Functions: Acts as a resource to ED staff and physicians regarding appropriateness of admission, levels of care (including related documentation requirements and observation vs. inpatient requirements), quality of care concerns and criteria/guidelines/protocols utilized in care planning and resource utilization. Gathers sufficient information from and communicates with all relevant sources to facilitate appropriate discharge from ED to appropriate level of care to assure it is done in an accurate, safe, timely and cost-effective manner to prevent readmission and/or frequent visits to ED. Assists in triaging calls from other acute care facilities requesting patient transfers to EMC; determines transfer appropriateness by reviewing requested documentation and InterQual level of care criteria and discussions as necessary with admitting physician, supervisors and/or EMC administrators and/or other EMC personnel. Assesses ED patients and identifies options other than acute hospital admission when appropriate. Screens and refers to acute rehabilitation, long-term acute care hospitals, and nursing homes for admission directly from the ED; screens and refers patients to clinics after initial exam; and screens and refers patients for whom treatments could be safely rendered at home with services (e.g., IV antibiotics, low molecular weight heparin injections, wound care, etc.). Organizes, integrates, and evaluates the effectiveness of the plan of care and progress toward achievement of desired outcomes. Modifies plan of care as patient/family needs change to accomplish goals established in the plan of care. Communicates plan of care, including changes and issues related to plan of care to patient/family, physicians, and other members of the healthcare team. Performs other job-related duties as assigned.
