Demonstrates succinct, legibly written documentation skills on all medical records and forms. Consistently documents pertinent information based on changes in patient condition/response and treatment provided. Navigates electronic medical record system to document completely and precisely. Participates in nursing research activities as requested. Formulates a goal-directed plan of care that is prioritized and based on patient diagnosis and anticipated patient outcomes. Individualizes updates and documents the plan of care. Includes the patient/family/designee (e.g. POA) in the planning process. Establishes priorities based on essential patient and family needs while incorporating available unit resources. Educates patients, families and caregivers appropriately on plan of care. Implements care in a thorough, safe, skillful, consistent and continuous manner. Delegates appropriately and coordinates duties of health care team members.
