As an integral member of the care team, they collaborate with other health care professionals to ensure continuity of care and coordination of a wide variety of patient care services. Responsibilities include coordinating patient communication, patient education, and preparing patients and/or caregivers for procedures and tests in collaboration with the health care team; ensuring continuity of care and coordination, identifying opportunities to improve patient care services; and assisting clinic management to identify and develop operational needs.
