Nurse Coordinator describes a variety of specialized nursing positions that provide professional support, planning, coordinating, and facilitating patient care for selected patient populations across the continuum of care, ensuring compliance with provider orders and established policies and procedures. This position utilizes the nursing process without providing direct patient care. Nurse Coordinators provide coordination and care for patients requiring assistance and support in accessing and managing their health care needs. As an integral member of the care team, they collaborate with other health care professionals to ensure continuity of care and coordination of a wide variety of patient care services. Responsibilities include coordinating patient communication, patient education, and preparing patients and/or caregivers for procedures and tests in collaboration with the health care team; ensuring continuity of care and coordination, identifying opportunities to improve patient care services; and assisting clinic management to identify and develop operational needs.

Camp Dates: 05/29-08/08 Shared housing and bathroom. Meals provided in the cafeteria Campers: All ages of individuals with disbilties (3y/o to adults) Assistance with personal care required for some campers. One day off per week.

The Registered Nurse is a clinical practitioner who coordinates and implements patient care specific to the age of the patient population served on the assigned units. He/she ensures that quality care is provided in an efficient and safe manner, consistent with the unit’s standards of care. He/she demonstrates performance consistent with the mission, philosophy and goals of the unit and organization. Demonstrates quality and effectiveness in work habits and clinical practice. Treats staff, physicians, patients and families with consideration and respect.

Provide professional nursing care to patients following established standards, practices and healthcare techniques. Demonstrate competence in making professional judgments pertaining to patient care. Assist providers in patient care and education. Actively participates as a member of the healthcare team to engage patients, families, and caregivers in the coordination of patient care.

Our staff of physicians, advanced practice providers and oncology-certified nurses is available to meet all our patients’ needs in the most comprehensive manner. Our goal is that patients and their families feel at home while receiving personalized, state-of-the-art care. Our laboratories are CLIA (Clinical Laboratory Improvement Amendments) and state certified, and also participate in the American Proficiency Institute’s Quality Control program.
