o Beds: 24 ? 16 beds for Hematology/Oncology ? 8 beds for Bone Marrow Transplant ? Avg daily census: 22 ? All same staff but BMT is positive pressure environment ? All RNs work interchangeably throughout 24 beds o Required Experience: 2 years Peds with strong Onc/Hem/BMT experience ? Nurses are acutely in the middle of tx, day 0, day 1 and during the sickest time – unless they are being intubated they are on this unit ? o Will you accept a first-time traveler? Yes if they are a strong BMT RN ? Patient Types/Common Diagnoses: o Ages: Neonate (more rare) to adults (if patient had childhood cancer treatment here, they will come to unit, even as an adult) o “On-Service”: Solid tumor cancers, Leukemia, Sickle cell, Aplastic anemia ? Highly acute bone marrow transplant population – this experience and skillset is a MUST – will receive patients s/p day 0, day 1 and 2 ? If child does not require intubation, they will come to this unit o “Off Service”: Can take off service (non-Hem/Onc) but must be without communicable infections/precautions due to the neutropenic patient population o Behavioral Health: unit will also commonly see BH Peds patients – diagnoses to include but not limited to: Pediatric behavioral/adjustment disorders, Suicidal Ideation, elopement, human trafficking ? Unit is equipped with cameras in care areas ? Psych Tech as available – will round a couple of times per shift and is always available if patient escalates ? Patient Ratios: o RN: Same on both shifts – acuity dependent ? Hem/Onc – 1:3 ? Observation/Off Service – 1:3-4 ? Bone Marrow Transplant – 1:1-2, based on acuity o CNA: 1:12 ? Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver’s License and/or Insurance: o BLS, PALS o APHON provider card ? APHON is just a certificate (like NIHSS) – CPHON and BMTCN are not required ? Skills required: o Required: ? Port access experience is a MUST, Bone marrow transplant experience with Day 0-2 is a MUST, Chemo administration, Blood product transfusion, cardiac rhythm interpretation ? Comfortability with blood cultures/central line care; PCA pumps, Heparin gtts o Preferred: ? End of Life care – Precedex gtt, Intransal Versed, Ketamine drips; chest tubes ? Support within the Department: o Charge RN/Supervisor – free floating o Unit Educator – Days – will serve as additional resource if needed o Task RN – 10am-1030p – will help with Admissions/Discharges/Tasks o Security – until 9:30p o HCTs – 1:12 ratio o Cameras within unit o Rounding Psych Tech as available – will round a couple of times per shift o PICU consult team (all Ketamine drips, etc) o APP (on site during day) o Provider on call overnight o IV/VAT team o Phlebotomy – able to assist with morning labs/peripheral draws if needed o RT, 24/7 in house o Telemetry monitoring – bedside/nurses station monitoring – nurses are responsible ? Technology/Equipment: o EMR: EPIC o IV Pumps: Alaris o Med Dispensing: Omnicell ? Floating: If yes, where? o General Pediatric Medial units – will be based on skillset and needs ? Ortho/Trauma, Gen Peds, Observation, ED Holds (admitted pts) ? No floating to Main ER/PICU/NICU

Children’s Hospital Los Angeles has two hepa-filtered inpatient units for hematology, oncology and BMT. The units are located on 4 East and 4 West, with 62 inpatient beds in total in the Marion and John E. Anderson Pavilion, 14 of which are in the Blood and Marrow Transplantation Unit. They are collectively comprised of approximately 85 percent oncology patients and 15 percent hematology patients.
