Nurses station is centralized in the middle, with the most critical patients being placed behind the nurses station Required Experience: 2 years Pediatric Cardiac ICU experience in a similar facility for acuity Will you accept a first-time traveler? Yes Patient Types/Common Diagnoses: Very high acuity patient population – ages 0-18 years old Patients with congenital heart differences that have undergone surgery, medical management of pre-transplant and post-transplant, s/p cath lab, ventricular assistive devices, ECMO, Impella, Dialysis, Aquadex (Renal Replacement Therapy) Patient Ratios: RN: Varies on acuity: 2:1, 1:1, 1:2 CNA: none – 1 NA per day to support unit secretary duties and nursing support needs throughout the shift Required Licensure & Certifications: BLS, PALS, NIHSS Skills required: Experience with fresh open heart post op patients; transplant experience, experience with the following devices: Aquadex (renal replacement therapy), Impella, ECMO EPIC experience Ability/Competency to float to PICU – will be discussed during interview Support within the Department: CNA? Duties? 1 Health Care tech per day to support unit secretary duties and nursing support needs throughout the shift (runners, bathing, etc.) Unit Secretary every shift Charge nurse – free floating Phlebotomy – in house but not utilized as majority of patients will have central lines IV/VAT Team? Yes Resource Nurse – free floating RT, 24/7 Cardiologist, 24/7 Intensivist, 24/7 Telemetry monitoring Tech? No Centralized and bedside monitoring Technology/Equipment: EMR: EPIC – required IV Pump: Alaris Med dispensing: Omnicell Monitors: Phillips Floating: If yes, where? Yes – PICU, NICU, Gen Peds – will float as needed

Beds: 24; 21-23 patient census on average Required Experience: 2 years of strong high acuity Pediatric ICU experience – must be comfortable in a high acuity, fast paced environment Will you accept a first-time traveler? Yes Unit Culture: Teamwork, Trust, Accountability, Respect, compassionately hard working Patient Types/Common Diagnoses: Respiratory, Trauma (which includes penetrating and non-accidental trauma, NAT), DKA, Seizures, Neurosurgical, Acute/Chronic Renal Failure, Orthopedic, Surgical, Transplants (Kidney and whole/partial Liver), complex Medical, Blood Disorders Patient Ratios: Weekend Rotation will be required to work 2 Fridays, 2 Saturdays, 2 Sundays and 2 Mondays every 6-week schedule ? The combination of the 2 Fridays, Saturdays, Sundays, and Mondays can be grouped together or worked separately as long as those eight (8) shifts occur within the 6-week period Holiday Requirements Travelers will work 75% of holidays, Unit requirement is the eve and day of every other holiday while contracted Floating Requirements o Pediatric CVICU, NICU, Neonatal PCU, Gen Peds – will float as needed

Medical management for patients with heart failure, Ventricular Assist, EMCO, arrhythmias and transplants Age range for neonates to young adults. Required Experience: PEDI CVICU, Ventalators, Cooling Blanket, IV Insertion, EPIC Charting Experience, High Frequency Vents and Oscillators, Nitric Oxide, Intracardiac Lines and Centra Line.
