POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge planning / Care progression / Communication with medical teams

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN License; BLS; 1 year HCA hospital case management experience; Meditech & Midas experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00AM to 4:30PM with one weekend/month & rotating holidays

SYSTEMS – Meditech/Midas

START DATE – ASAP
