OT needed asap for 13 weeks
M-F 9-5 Occasional 6-7am start time for morning ADLs. One Saturday a month
Facility Setting? Inpatient rehab
Number of beds in your facility? 16
Preferred experience level? 1 year
EMR for your facility: Cerner
Scrub colors / dress code: Grey scrub top and black scrub bottoms
Parking Location: Onsite

REQUIREMENTS

Occupational Therapist, Acute
Experience: 1 year
Licenses: OT-NM
Certifications: OTR-National Board for Certification in Occupational Therapy(Copy needed)
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year
References: 1 Reference in entire work history
