POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – NC/compact RN license; BLS; 2 years acute hospital case management experience; Work collaboratively with RNCM; Documentation & communication skills; Ability to shift attention between tasks & to build trust with team/patients and family

LENGTH OF ASSIGNMENT – 26 weeks

SHIFT / HOURS PER WEEK – 8:00AM-5:00PM

SYSTEMS – Cerner, Nate, Ensocare

START DATE – 3/31/2025
