POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – NH/compact RN license; BLS; 1 year acute hospital case management experience

PREFERRED QUALIFICATIONS – Home Health/Hospice Experience; 1 Year bedside RN experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00 AM – 4:30 PM & every other weekend

START DATE – ASAP
