JOB DETAILS

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge Planning evaluations and re-evaluations, including follow-up; 30 Day readmit evaluations; Arranging post-acute care needs; Completing InterQual reviews daily

MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 2 years acute hospital case management experience; Meditech & MIDAS experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00 am - 4:30 pm

SYSTEMS – Meditech / Midas

START DATE – ASAP
