POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; AHA BLS; 2 years recent RN Case Management & discharge planning experience in a hospital setting

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Monday - Friday 9a-5:30p. Weekends on request.

START DATE – 4/14/2025
