POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – ME/compact RN license; BLS; 2 years acute hospital case management experience

PREFERRED QUALIFICATIONS – EPIC, utilization review, discharge planning, and care coordination of patients from the ED, ICU, and Med-Surg units experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7:30 AM-4:00 PM
