JOB DETAILS
POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – WA RN license; AHA BLS; 2 years RN Case Management experience, with experience in a long-term acute care setting; 6 hours of Suicide Prevention Training completed through the WA DOH website.
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – Monday - Friday 8a-430p (40hrs/wk)
START DATE – 4/21/2025
