POSITION SUMMARY – RN - Case Manager/UR

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – 1.5+ years recent case management experience (within past 2 years) in an acute care facility; EPIC experience; NY RN license; BLS

PREFERRED QUALIFICATIONS – ED CM experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8hr days
