POSITION SUMMARY – RN/LPN Care Manager

POSITION DUTIES – Assess all admission referrals from acute care settings or other facilities. Evaluate patients' skilled/Long Term Care requirements, verify eligibility benefits, and ensure alignment with organizational strategic goals before admitting them.

MINIMUM REQUIRED QUALIFICATIONS – ME/compact RN or LPN license; BLS/BCLS; 3 years Care Coordinator/Care Manager experience

PREFERRED QUALIFICATIONS – Experience working with Geriatrics patients

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Days

START DATE – ASAP
