POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Utilization Management & Discharge Planning; Dealing with all payers, uninsured, homeless. Completing Initial assessments, IQ, Cerner; First to float

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BSN; BLS; 2 years acute hospital case management experience; Prior Dignity experience

PREFERRED QUALIFICATIONS –

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8hr Days, EOW

SYSTEMS – Cerner / Interqual

START DATE – 4/15/2025
