JOB DETAILS

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge Planning, SNF referrals, LTAC, HH, NH, HD placement, DME, Hospice, Family meetings, large unfunded and under- resourced population

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 1 year acute hospital case management experience

PREFERRED QUALIFICATIONS – Meditech, MIDAS, NATE experience

LENGTH OF ASSIGNMENT – 8 weeks

SHIFT / HOURS PER WEEK – 8 Hours Mon-Friday, would accept 12 hour shift staff, including every other weekend if available

SYSTEMS – Meditech / Midas

START DATE – 4/21/2025
