POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Utilization Management / Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – NY RN license; AHA BLS; 2 years case/resource management and utilization management

PREFERRED QUALIFICATIONS – 3-5 years Care Management experience; EPIC experience; PRI (Patient Review Instrument); SCREEN experience/certification

LENGTH OF ASSIGNMENT – 9 weeks

SHIFT / HOURS PER WEEK – 9:00 AM-5:00 PM M-F
