TYPE OF JOB ORDER: RN Case Manager
START DATE: ASAP
REQUIRED SKILLS: 2 years minimum discharge planning experience; strong acute care hospital experience- 

M-F with one weekend every 5-6 weeks

EXPECTED HOURS: 40
LICENSE/CREDENTIALS REQ: DC license - can apply upon offer
SYSTEMS: Cerner experience required
